Your Done-For-You State Licensing Application is prepared by Michele Lee Ellis Consulting, LLC, through its Home Care Coaching brand (“we,” “us,” or “our”), to help you submit a complete and accurate application based on the information, documentation, and cooperation you provide.

If you are not awarded your license solely because of an error or omission we made in performing the services included in your Done-For-You State Licensing Application, and we are unable to correct that error or omission after receiving a reasonable opportunity to do so, we will refund the fees you paid to us for that application service.

To qualify for this guarantee, you must:

• Provide complete, accurate, and timely information and documentation, and fulfill all applicant responsibilities and state requirements.

• Provide us with complete copies of all communications and materials received from the state licensing agency within 24 hours of receipt, including notices, emails, attachments, requests for information, deficiency letters, deadlines, and licensing decisions. Communications requiring action sooner than 24 hours must be forwarded immediately.

• Complete all requested applicant corrections and provide any necessary information, signatures, or authorizations within the deadlines communicated to you.

• Notify us in writing of the issue and allow us a reasonable opportunity to review and correct any error or omission, including responding to the state or submitting corrected materials when permitted. This correction process does not extend state deadlines.

This guarantee does not apply when your failure to obtain a license results, in whole or in part, from your decision to stop or withdraw from the process; failure to provide required information or state communications on time; incomplete or inaccurate information; failure to complete your responsibilities or requested corrections; or failure to cooperate with our efforts to correct an issue.

This guarantee also does not cover licensing outcomes caused by applicant ineligibility, failure to meet state requirements outside the scope of our services, or other circumstances beyond our control.

Any qualifying refund is limited to the fees paid to Michele Lee Ellis Consulting, LLC for the Done-For-You State Licensing Application service. State application fees, third-party expenses, and fees for other services are excluded.

The state licensing agency makes the final licensing decision. We do not guarantee approval or a specific approval date. Nothing in this guarantee limits rights or remedies that cannot lawfully be limited.
